In accordance with the Health Insurance Portability and Accountability Act (HIPAA), Open Fields Counseling may use your protected health information (PHI) for treatment, payment, and health care operations. We may also use or disclose your information for other purposes permitted or required by law.
You have the right to:
Request restrictions on certain uses and disclosures
Receive confidential communications
Inspect and obtain a copy of your health record
Request an amendment to your health record
Receive an accounting of disclosures
Obtain a paper copy of this notice upon request
We are required by law to maintain the privacy of your PHI, provide you with this Notice, and notify you if a breach occurs.

Good Faith Estimate Notice (No Surprises Act)
Effective Date: 7-1-2025Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services.
You can ask for this estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill.
